Find your new doctor's name in Section 4 of the Patient Enrollment Form you received in the mail.

1) PATIENT ENROLLMENT FORM

Please complete the Patient Enrollment form. Complete one form for each adult family member (16 years or older) who wishes to enroll. Children and dependent adults can be added to a single form in section 2 of the form. If you need additional copies of the form, you can download them from the links provided below.

Each family has already been assigned to one of the four physicians in Advancing Care Medical Clinic. To determine the doctor to whom your family has been assigned, you can find the name in Section 4 of the Patient Enrollment form that you received in the mail.

The Ontario Ministry of Health requires that the form be printed and signed. Please don't forget to sign the form before you choose one of the following methods to submit it:

a. Scan or take photos of all forms and email to: advancingcare@rsrs.com
b. Send the completed forms by FAX to: (416) 398-5932
c. Mail the completed forms to: Advancing Care Medical Clinic c/o RSRS, 111 St. Regis Cres. S., Toronto, ON M3J 1Y6
d. Bring the forms to your first appointment with Advancing Care Medical Clinic (the office will also have copies of blank forms, if required)

(Please note that your spot in Advancing Care Medical Clinic is only guaranteed if you enroll by June 1st, 2018 as they will resume enrolling their own patients thereafter.)

2) MEDICAL RECORD REQUEST FORM

The Medical Record Request form must be completed to ensure your record is available to Advancing Care Medical Clinic. (Please note that the fee for preparation and transfer of your record is not covered by OHIP). You can use the simple online form (or complete the paper form you received in the mail and send it in along with the Patient Enrollment form).

RSRS will access your record(s) and will send a copy(ies) to Advancing Care Medical Clinic and/or to you. If you are not enrolling with Advancing Care Medical Clinic, you should still use the form to request a copy for yourself.
